Recognising diabetes educators who are changing lives
Life with type 1 diabetes can be complicated, demanding and stressful. JDRF recognises that diabetes educators can change lives through caring professional advice that achieves improved health and wellbeing.
JDRF is proud to deliver with the support of Abbott Diabetes Care the 2011 Diabetes Educator of the Year Awards.
The Diabetes Educator of the Year Awards acknowledge and thank Credentialed Diabetes Educators, Clinical Nurse Consultants or Clinical Nurse Educators for their exceptional contribution and dedication. Each year, three professionals will receive a thanks and recognition, a trophy and cash prize of $3000 to be spent in clinic to assist in patient care.
Who can nominate a diabetes educator?
Members of the type 1 diabetes community are encouraged to nominate diabetes educators that have changed their lives. Diabetes educators can also nominate themselves or their colleagues.
Three Diabetes Educator of the Year Awards
- Excellence and Innovation - Exceptional or innovative product, program, process or approach to patient care e.g. support and education sessions, education booklet, patient newsletter, new transition program OR Participation and translation of innovative training or research
- Impact and Relationships - Participation in advocacy aimed to improve the outcomes for the type 1 diabetes community as a whole eg political advocacy, fundraising, community involvement, policy improvements OR Outstanding relationship development and communication that resulted in exceptional patient psychological or social outcome.of innovative training or research into outstanding patient outcomes
- The People's Choice Award (Patient or family nominations only) - Patients can nominate an educator that has proven to work above and beyond the standard delivery of health care.
Who judges the Diabetes Educator Awards?
Awards are decided by an independent review panel consisting of health care professionals and members of the type 1 diabetes community.
Timelines
Nominations open: 31th August 2011
Nominations close: 30th November 2011
Winners announced: 24th February 2012
Awards Presentation: The Diabetes Educator of the Year Awards 2011 will be presented at the JDRF Gala Ball in Sydney in May 2012. All winners and their partners will be invited to attend the Ball as special guests. Also provided: Flights to Sydney and one night's accommodation for each winner and their partner.
